Mistake by a doctor
As a doctor I was extremely shocked and surprised to watch the TV coverage of Modern Diagnostic Center, one of the well reputed diagnostic centers in the city some days ago. In this regard I have some points to make:
1. Reports of Ultrasonography of the whole abdomen or pelvis are to be framed in different proforma- male/female. The sinologist usually dictates the findings and the typist completes the report and gets it signed by the Sonologist. The mistakes at the clinic level were-
a) May be the clinic did not have the different proforma as said.
b) Typist by mistake made the report on the wrong proforma.
c) The Sonologist, being a doctor, should not have singed the report without going through it.
The referring physician on finding the wrong report could have phoned the doctor concerned or the medical centre to make a fresh report and asked them to be extra careful in future.
